To Make a Referral

Cherrybrook Care Home offers a flexible and clinically responsive approach to both long-term placements and shorter-term step-down care. Our team is experienced in supporting adults with complex nursing needs, neurological conditions, dementia and enduring mental health diagnoses.

We work closely with referring professionals, families and the individual to ensure that Cherrybrook is the right fit and that the transition into our care is smooth and well-managed. Following receipt of a referral, our Clinical Team will carry out a prompt and thorough assessment, either in person or remotely, depending on the individual’s location and needs. This collaborative process ensures continuity of care and supports a safe, person-centred admission pathway.

Specialist Support for Complex Care Needs

At Cherrybrook Care Home, we provide a clear and coordinated care pathway for adults with complex nursing needs, neurological conditions, specialist dementia and long-term mental health challenges. Our approach is person-centred, collaborative and outcomes-focused, supporting each individual to live with dignity, purpose and the right level of support for their needs. We aim to create a positive environment that is inclusive, respectful and built around each person’s goals and lived experience.

Complex Nursing Care

Cherrybrook offers round-the-clock nursing care for adults who require clinical oversight and ongoing support with complex physical health needs. Our team includes registered nurses on-site 24 hours a day, supported by experienced care staff, enabling us to manage a wide range of long-term conditions including mobility impairment, respiratory illnesses, neurological disorders and post-operative recovery.

Referrals are appropriate for individuals who are medically stable but require continued monitoring, medication management, and personal care that cannot be safely provided in a non-nursing or home environment. Our team works closely with GPs, community health teams and families to ensure continuity of care, with a focus on maintaining comfort, dignity and stability.

Nursing Care

Our dementia care service is suitable for individuals at all stages of dementia, including Alzheimer’s disease, vascular dementia, frontotemporal dementia and mixed forms. Cherrybrook provides a secure, structured environment with experienced staff trained in managing behaviours that may challenge, supporting cognitive decline, and providing non-pharmacological interventions.

We accept referrals for individuals who require 24-hour support in a dementia-informed setting. The environment is designed to promote calm and reduce disorientation, with access to sensory spaces and therapeutic activities. We also work in partnership with community mental health and memory services to ensure care is responsive to changing clinical needs.

Mental Health Support

Cherrybrook provides long-term and step-down care for adults living with complex or enduring mental health conditions. We support individuals with diagnoses such as schizophrenia, bipolar disorder, personality disorders and mood-related conditions, including those requiring structured routines and behavioural support.

Our team includes registered nurses and trained carers with experience in mental health, working alongside external professionals such as community mental health teams, psychiatrists and crisis services. Referrals are appropriate for individuals whose needs cannot be safely met at home but who do not require hospital-level intervention. Care plans are person-centred, risk-managed and built around therapeutic engagement.
